Job summary

HomeFirst is seeking a Temporary Overnight Case Manager for the Washington Community Center in San Jose. You will provide direct case management to adults experiencing homelessness, conduct assessments, assist with benefits, safety planning, and crisis de-escalation during the 2pm–11pm swing shift.

Responsibilities emphasize documentation in HMIS, collaboration with community partners, and adherence to Housing First and Harm Reduction principles.

Qualifications

• Minimum two years’ experience in social services
• Bachelor’s degree in psychology, counseling, mental health, human services or related field acceptable
• Lived experience or understanding of unhoused population
• Fluency with Microsoft Office Suite

Responsibilities

• Provide case management services including enrollment, ongoing needs and status assessments, budgeting, information and referral
• Crisis de-escalation, safety planning, risk assessment and mandated reporting
• Documentation in HMIS within 3 business days and maintain all case file paperwork
• Attend meetings and training relevant to this position and assist with office tasks
