Job summary

HomeFirst is seeking a Temporary Case Manager for the Overnight Warming Location in San Jose, CA. The role supports ending homelessness through Housing First and Harm Reduction approaches, focusing on crisis response and housing stability.

The position reports to the Program Manager, with a Swing Shift (2pm-11pm). Employment is temporary for the program term through April 2027, with full-time benefits and a comprehensive set of services provided by HomeFirst.

Qualifications

• Minimum two years of social services experience; bilingual a plus.

Responsibilities

• Direct Services: provide case management, enrollment, needs assessments, budgeting, information and referral, crisis de-escalation, safety planning, employment and benefits assist, advocacy, and collaboration with partners.
• Documentation: record activities in HMIS within 3 business days and maintain required paperwork; review program practices and improve service quality.
• General: attend meetings, assist with office tasks, and support other duties as assigned.
