Job summary

HOUSING WORKS INC is seeking a Community Liaison in New York to enhance the client experience across Housing Works Health Center programs. You will support enrollment, access to entitlements and benefits, and coordinate transportation for clients in a fast-paced setting.

The ideal candidate has 1–2 years in community health settings, excellent verbal and written communication, and the ability to work both independently and with a team.

Qualifications

• High School degree or higher; degree preferred or in progress.
• 1–2 years in a community-based setting including health services.
• Customer service skills with problem-solving.
• Organized and able to multitask in a fast-paced environment.
• Able to work independently while also supporting the team.
• Excellent verbal and written communication; phone etiquette and concise emails.
• Exceptional interpersonal skills; professional and diplomatic.
• Accurate, precise and timely in reporting and admin tasks.

Responsibilities

• Maintain knowledge of Housing Works' programming, screening tactics and eligibility requirements and articulate benefits to eligible clients/patients.
• Assist in enrolling clients or registering patients in programming and help with access to entitlements and benefits.
• Coordinate client transportation and ensure adherence to visits/appointments.
• Navigate site during day of appointment and deliver courtesy lunches.
• Provide follow-up to ensure adherence to referrals via calls or emails.
• Document and think through solutions for customer service issues.
• Assist in organizing outreach and producing data reports to improve partnerships.
• Meet weekly/monthly reporting requirements for client engagement and enrollments.
• Participate in training and development opportunities, including certifications.
• Be an active member of the Community Partnership team in weekly huddles and meetings.
