Job summary

Catholic Community Services and Catholic Housing Services is seeking an Outreach Case Manager to support Housing and Essential Needs in Thurston County. You will conduct HEN intakes at multiple sites, provide case management, and help clients access housing resources.

Responsibilities include coordinating with entry providers, crisis intervention, data entry into HMIS, and maintaining detailed case notes and records for program compliance.

Qualifications

• Post-secondary education in a social services field or a minimum of 2-year experience working with individuals and/or families with complex needs.
• Applicant must successfully pass required background checks prior to an offer of employment.
• Understanding of problems homeless people often face, including alcohol and drug abuse, mental illness, domestic violence, and long-term homelessness.
• Excellent written and oral communication.
• Good office computer skills, with ability to maintain complex client records and update databases including HMIS.

Responsibilities

• Completes HEN Program intake.
• Coordinate with the local Coordinated Entry provider to prioritize clients by vulnerability.
• Answer telephone and conduct interviews to determine HEN eligibility.
• Provide Case Management services for clients seeking housing and resources.
• Create client-directed service plans and transport clients to housing opportunities.
