Job summary

Catholic Community Services is seeking an Outreach Case Manager for the Housing and Essential Needs program in Thurston County, WA. You will perform HEN eligibility intakes, coordinate with entry points, and provide case management and crisis support to clients transitioning to housing and resources.

The role emphasizes data entry in HMIS, collaboration with community partners, and developing client-centered service plans to improve access to housing and essential needs.

Qualifications

• Post-secondary education in a social services field or 2+ years of relevant experience.
• Pass required background checks before job offer.
• Understanding of homelessness-related challenges including mental health and substance use.
• Excellent written and verbal communication abilities.
• Ability to maintain client records and HMIS data accurately.
• Collaborative, culturally competent work approach.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Complete intake for the HEN program and coordinate with Coordinated Entry.
• Conduct phone interviews to assess eligibility for the HEN program.
• Provide case management and crisis intervention services.
• Develop client-directed service plans and track progress.
• Establish additional intake sites for better access across Thurston County.
• Ensure CHG compliance and maintain accurate documentation.
