Job summary

Catholic Community Services seeks an Outreach Case Manager to support Housing and Essential Needs (HEN) in Thurston County. You will complete HEN intakes, coordinate with agencies, and provide case management and crisis intervention to clients facing housing challenges.

You will maintain accurate records, collaborate with a multidisciplinary team, and help clients transition to stable housing and resources. Training provided; strong empathy needed.

Qualifications

• Post-secondary education in a social services field or minimum of 2-year experience with individuals and/or families with complex needs.
• Background checks required prior to employment.
• Understanding of problems homeless people face, including addiction, mental illness, domestic violence, and long-term homelessness.

Responsibilities

• Completes intake for the HEN Program.
• Coordinate with local Coordinated Entry to place clients into priority pools based on vulnerability.
• Answer calls and conduct interviews to determine HEN eligibility.
• Provide Case Management and Crisis Intervention services for clients seeking housing and resources.
• Create client-directed service plans and transport clients to housing opportunities.
• Advocate on behalf of clients and connect them to resources to overcome barriers.
• Maintain client files, records, and HMIS data accurately.
• Establish additional intake sites within Thurston County for easier access to HEN benefits.
