Job summary

Catholic Community Services in Bremerton, WA is seeking an Outreach Case Manager to join the Housing and Essential Needs team. You will conduct HEN program intakes, coordinate with Coordinated Entry, and provide case management and crisis intervention to help clients secure housing and resources.

This role requires strong communication, ability to maintain records, data entry into HMIS, and driving to different Kitsap County sites.

Qualifications

• Post-secondary education in a social services field or 2+ years working with individuals/families with complex needs.
• Excellent written and oral communication.
• Good office computer skills; maintain complex client records and update databases including HVIIS.
• Commitment to CCS mission and values.
• Able to transport clients in personal vehicle; valid license and insurance.

Responsibilities

• Complete HEN intake at primary office and across Kitsap County sites.
• Coordinate with Coordinated Entry to place clients into priority pools based on vulnerability.
• Answer phones and conduct intake to determine HEN eligibility.
• Provide case management and crisis intervention services for clients seeking housing/resources.
• Create client-directed service plans and transport clients to housing opportunities.
• Maintain comprehensive client files with timely notes and documentation.
• Input data into HMIS and prepare rent requests and orders as needed.
