Job summary

Catholic Community Services is seeking an Outreach Case Manager for the Housing and Essential Needs (HEN) program in Kitsap County. The role focuses on intake, eligibility, and connecting clients with housing resources while maintaining thorough case records and HMIS data entry.

The position supports crisis interventions, collaborates with local entry providers, and builds accessible intake sites across Kitsap County to help clients in Bremerton and Poulsbo.

Qualifications

• Post-secondary education in a social services field or a minimum of 2-year experience working with individuals and/or families with complex needs.
• Excellent written and oral communication.
• Mental health and/or crisis intervention experience.
• Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.

Responsibilities

• Complete intake for the HEN Program.
• Coordinate with the local Coordinated Entry provider to prioritize clients.
• Answer telephone and conduct interviews to gather eligibility information.
• Provide Case Management and Crisis Intervention services for clients.
• Maintain client files and timely case notes with documentation.
• Input data into HMIS in a timely manner.
