Job summary

Advocate Health Care in Elgin, IL is seeking a part-time Speech Language Pathologist to evaluate and treat patients with a range of speech, language, voice, cognitive and swallowing disorders.

The role requires CCC SLP and IL licensure, with responsibilities including developing plans of care, documenting progress, coordinating with physicians and care teams, and preparing patients for discharge with home programs.

Qualifications

• Master's degree in Speech Pathology from an accredited program.
• CCC SLP license and IL state licensure required.
• BLS for Healthcare Providers required.
• Outpatient experience preferred.
• Competencies in hospital-based standards and safety.

Responsibilities

• Interprets physician referrals and conducts speech and language evaluations for diverse disorders.
• Establishes a written plan of care and implements individualized treatment.
• Monitors progress and adjusts treatment; communicates with physicians and team.
• Completes documentation and billing per department policy and accreditation standards.
• Discharge planning and family/patient education; transition planning within care continuum.
