Job summary

Advocate Health in Downers Grove, IL seeks a part-time Speech Language Pathologist to evaluate and treat patients with speech, language, voice, cognitive and swallowing disorders. You will develop written plans of care and monitor progress, adjusting therapy as needed, with discharge planning and family education included.

Responsibilities include collaboration with the care team, accurate documentation, and adherence to safety and infection control standards.

Qualifications

• Master’s degree in Speech Pathology from an accredited school.
• Licensed Speech Language Pathologist in the state of practice.
• Basic Life Support (BLS) for Healthcare Providers.
• Certificate of Clinical Competence (CCC-SLP) from ASHA.

Responsibilities

• Interprets physician referrals and conducts evaluations for speech, language, voice, cognitive and swallowing disorders.
• Establishes a written plan of care and implements treatment based on evaluation results and goals.
• Monitors outcomes, adjusts treatment plans, and provides updates to physicians and team.
• Plans for discharge including home programs, family teaching, and referrals.
• Communicates with stakeholders to ensure optimum patient care.
• Documents evaluations, progress notes, and discharge summaries per policy.
