Qualifications

• Bachelor’s degree in social work, psychology, human development or related field preferred.
• Experience in coordinating outpatient services in mental health or substance use is beneficial.
• Strong written and verbal communication skills required.

Responsibilities

• Coordinate services for patients receiving Medication Assisted Treatment (MAT).
• Communicate with referral sources and primary care about progress and planning.
• Assist in scheduling treatment and arranging transportation when needed.
• Develop individualized treatment plans and monitor progress with the clinical team.
• Represent the agency professionally and link families with community resources.
