Job summary

Carle Health is seeking a licensed Physical Therapist to plan and administer PT treatment plans that alleviate pain, restore function, and promote overall health. The role requires pediatric and adult patient care in an outpatient setting, with a leadership and teaching component for staff and students.

Responsibilities include designing care plans, adjusting based on patient response, educating patients and families, and collaborating with a multidisciplinary team.

Qualifications

• Licensed Physical Therapist (PT) per state requirements.
• Basic Life Support (BLS) within 30 days - American Heart Association.

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes and discharge plan.
• Educates patients, family, caregivers and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examinations and selects reliable methods relevant to chief complaint and history.
• Monitors care and provides feedback to support personnel and team members.
• Communicates with the medical team and documents per department standards.

Bachelor's Degree: Physical Therapy (Required)
