Job summary

Qualifications

• Licensure as a Physical Therapist (PT) with IDFPR licensure (Illinois) or equivalent.
• Basic Life Support (BLS) certification within 30 days - American Heart Association (AHA).
• Bachelor's Degree in Physical Therapy is required.

Responsibilities

• Demonstrates patient transfer techniques with assistance as needed.
• Maintains safe body mechanics to protect patients and staff.
• Designs and manages a plan of care including goals, outcomes, and discharge plan.
• Adjusts care based on patient response and evidence.
• Educates patients, families, and other providers on treatment plans and expectations.
• Evaluates clinical findings to determine diagnoses and outcomes of care.
• Performs examinations using appropriate methods and screening.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with the medical team and documents care per standards.
• Meets customer service and productivity standards and serves as clinical instructor when assigned.
