Qualifications

• Licensed Physical Therapist (PT) in Illinois as per IDFPR.
• Basic Life Support (BLS) within 30 days per AHA.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Maintains safe body mechanics to protect patients and staff.
• Designs and updates plans of care with goals, outcomes, and discharge plan.
• Adjusts care based on patient response and evidence.
• Educates patients, families, and other providers using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes.
• Performs examinations and selects appropriate assessment methods.
• Screens patients to determine need for therapy services within legal/ethical guidelines.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with the medical team and documents care.
• Meets customer service and productivity standards.
• Serves as clinical instructor when assigned.
