Job summary

Carle Health is seeking a licensed Physical Therapist to plan and administer treatment plans that alleviate pain, restore function, prevent disabilities, and promote overall fitness and health. You will serve as a clinical resource and model for staff and students.

The role emphasizes outpatient orthopedic care with ongoing patient education and teamwork within a multidisciplinary setting. A Bachelor’s Degree in Physical Therapy is required and Illinois PT licensure is expected with BLS

Qualifications

• Licensed Physical Therapist (PT) per IDFPR
• BLS within 30 days by AHA
• Bachelor’s Degree in Physical Therapy (Required)

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and screens patients to determine needs for therapy services and treatment.
• Monitors care delivered by support personnel and provides regular feedback.
• Communicates with the medical team and documents care per department standards.
• Acts as a clinical instructor when assigned and participates in team meetings.
