Qualifications

• Licensed Physical Therapist (PT) by Illinois Dept. of Financial and Professional Regulation (IDFPR)
• Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
• Education: Bachelor's Degree: Physical Therapy (Required)
• Work Experience: Outpatient orthopedic

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Maintains safe body mechanics to protect patients and staff and treats everyone with dignity and respect.
• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge planning.
• Adjusts the care based on patient response and evidence.
• Educates patients, family, caregivers, students, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination using reliable methods relevant to chief complaint and medical history.
• Screens patients to determine the need for therapy services according to standards.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with medical team and documents per standards.
• Meets customer service and productivity standards.
• Serves as clinical instructor when assigned and participates in team activities.
• Performs reporting and other duties as assigned.
