Job summary

Carle Health in Illinois seeks a licensed Physical Therapist to plan and administer treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health. The role serves as a clinical resource for staff and students and collaborates with the medical team in outpatient care.

You will educate patients and families, perform evaluations, develop care plans, supervise students when needed, and document care in line with department standards.

Qualifications

• Licensed Physical Therapist (PT) with IDFPR license.
• BLS certification within 30 days as required by AHA.

Responsibilities

• Demonstrates ability to assist patient transfers from bed, chair or floor as needed.
• Maintains safe body mechanics to protect patients and staff, treating everyone with dignity.
• Designs a plan of care combining goals, treatment, outcomes and discharge planning.
• Adjusts care based on patient response and evidence.
• Educates patients, families and caregivers using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes.
• Performs examinations using reliable methods aligned with chief complaint and history.
• Screens patients to determine need for therapy services following legal and ethical standards.
• Monitors care delivered by support staff and provides feedback.
• Communicates with the medical team and documents per standards.
• Meets customer service and productivity expectations.
• Serves as clinical instructor when requested by leadership.
• Participates in team, program and department activities and meetings.
• Performs reporting and other duties as assigned.
