Job summary

Carle Health is seeking a licensed Physical Therapist to plan and administer treatment plans for outpatient orthopedic patients. You will serve as a clinical resource, design and adjust care plans, educate patients and care teams, and ensure high-quality documentation and teamwork.

Qualifications

• Licensure as Physical Therapist (PT) in Illinois and Basic Life Support (BLS) within 30 days per AHA guidelines.
• Bachelor's Degree in Physical Therapy is required for this outpatient role.

Responsibilities

• Demonstrates patient transfers with assistance as needed.
• Maintains safe body mechanics to protect patients and staff.
• Designs and adjusts a plan of care with goals, treatment, outcomes and discharge plan.
• Educates patients, families and other health care providers using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes of care.
• Performs examinations and selects appropriate assessment methods.
• Screens patients to determine the need for therapy services.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with the medical team and documents in the system.
• Meets customer service and productivity standards.
• Serves as clinical instructor when assigned, and participates in team activities.

Education

Bachelor's Degree: Physical Therapy (Required)
