Job summary

Carle Health is seeking a qualified Physical Therapist to plan and administer treatment plans that alleviate pain, restore function, prevent disability, and promote overall fitness and health. This role serves as a clinical resource and educator for staff and students within an outpatient orthopedic setting.

The position requires a valid PT license (IL), BLS within 30 days, and a Bachelor's degree in Physical Therapy.

Qualifications

• Licensed Physical Therapist (PT) – Illinois is required.
• Basic Life Support (BLS) within 30 days is required.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history.
• Monitors the care delivered by support personnel and provides regular feedback.
• Communicates with medical team.
• Performs documentation according to department and practice standards.
