JAG Physical Therapy is seeking a forward-thinking Physical Therapist to assess patients, devise creative treatment plans, and maximize functional independence. You will work in a high-energy environment, maintain confidentiality, and collaborate with a team to deliver quality care.

Qualifications

• Bachelor's or Master's degree required; Doctorate preferred.
• Licensed Physical Therapist in the state.
• CEU requirements completed in appropriate state (NY, NJ).
• Experience with EMR software preferred.
• Excellent oral and written communication.

Responsabilités

• Collect and record patient data per guidelines and payer requirements.
• Perform initial evaluation, daily notes, progress reports, and discharge plans.
• Record daily billing and maintain productivity per guidelines.
• Prepare for evaluations per department policies and practice standards.
• Select and administer tests and measures; formulate diagnosis and plan of care.
• Establish treatment plan and collaborate with the interdisciplinary team.
• Identify need for consultations and discharge planning; educate patient and family.
• Supervise PTA, ATC, PT aides; participate in seminars and community outreach.
