Qualifications

• Bachelor’s or Master’s Degree required; Doctorate degree preferred.
• Licensed Physical Therapist in NY practicing state.
• Completed CEU requirement in NY/ NJ.
• Valid CPR license.
• Experience/knowledge with an EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision-making skills.
• Excellent work ethic and dedication to patient success.
• Basic computer skills.
• Team-oriented with ability to work independently.

Responsabilités

• Collect and record patient data per guidelines and reimbursement needs.
• Perform initial evaluation, daily notes, progress reports, discharge plans and team communication.
• Record daily billing and follow productivity guidelines.
• Prepare for evaluations per department policies and standards.
• Select and administer tests and measurements.
• Synthesize results to form diagnosis, goals and plan of care.
• Establish and adjust treatment plans with interdisciplinary teams.
• Refer to other health professionals when needed and complete discharge plans.
• Educate patients and families; train in ADLs/IADLs and rehab equipment.
• Provide musculoskeletal interventions and modalities.
• Document treatments in Prompt EMR.
• Oversee PTA, ATC, and PT aides; participate in seminars and community outreach.
