SourcePro Search, LLC is seeking an experienced Outpatient Physical Therapist for a full-time role in Cayuga, NY, within the Finger Lakes region. You will provide hands-on rehabilitation in an outpatient clinic, evaluating patients and designing individualized plans.

The role emphasizes collaboration, ongoing professional development, and a supportive team in a premier health organization; candidates must have a DPT, NY license eligibility, and BLS certification within 90 days.

Qualifications

• Experience in outpatient physical therapy settings is preferred.
• Must hold or be eligible for a valid Physical Therapist license in New York.
• Doctor of Physical Therapy (DPT) degree from an accredited program is required.
• BLS certification required within 90 days of hire.
• Ability to effectively communicate and collaborate with patients, families, and team members.
• Deliver high-quality, compassionate care for a wide range of conditions.
• Must be able to lift up to 30 pounds independently and up to 150 pounds with assistance.

Responsabilités

• Conduct initial evaluations and develop individualized treatment plans.
• Administer therapies including exercises, manual therapy, modalities, and education.
• Educate patients on exercise programs and injury prevention.
• Collaborate with the multidisciplinary care team for comprehensive care.
• Monitor progress and adjust treatment plans as needed.
• Document plans, progress notes, and discharge summaries per policy.
• Provide excellent patient education to empower self-management.
• Maintain a clean, safe, and organized treatment environment.
