Responsibilities

• Evaluates patients, selects and administers assessment techniques, synthesizes information to establish a patient care plan.
• Gathers data from medical records, physician reports and patient/family with thorough documentation.
• Establishes functional goals and provides evidence-based interventions to meet them with ongoing plan-of-care adjustments.
• Documents evaluations, tests, progress notes, treatment notes, education, outcomes and discharge notes.
• Communicates evaluations and progress to physicians and other clinical staff; coordinates patient schedules and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
