Job summary

Advocate Health's Atrium Health Physical Therapy University City is seeking a licensed Physical Therapist for full-time outpatient rehabilitation. The role involves comprehensive patient evaluation, planning and implementing evidence-based therapies, and coordinating care with physicians and the rehab team.

Responsibilities include documenting progress, establishing goals with patients, supervising support staff when required, and ensuring discharge planning is addressed.

Qualifications

• Bachelor's degree in Physical Therapy (required).
• Applicable state license for Physical Therapy.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients and establishes care plans with modalities, exercises, and education.
• Gathers data from medical records and patient/family and documents accordingly.
• Sets functional goals with patients and provides evidence-based interventions with ongoing reassessment.
• Documents evaluations, tests, progress notes, discharge notes and charges as needed.
• Communicates evaluation and progress to physicians and care team; coordinates schedules and discharge planning.
• Supervises PTAs and Rehab Tech/Aide as required for licensure.
