Job summary

Advocate Health, in collaboration with Atrium Health Cleveland Outpatient Rehabilitation, is seeking a full-time Physical Therapist to evaluate patients, develop plans of care, and supervise rehab staff. You will gather data, set functional goals, and provide evidence-based interventions with ongoing assessment to optimize outcomes.

You must hold a Bachelor's in Physical Therapy with an active state license and BLS.

Qualifications

Bachelor's degree in Physical Therapy with an active state license is required.
BLS certification is required per policy guidelines.
36 hours per week in a full-time schedule; M-F with 36 hours.

Responsibilities

Evaluates patients and selects/ administers assessment techniques to formulate care plans.
Gathers data from medical records, physician reports, patients and families.
Establishes functional goals and provides evidenced-based interventions with ongoing reassessment.
Documents evaluations, treatments, progress notes, and charges as needed.
Communicates progress to physicians and staff; coordinates schedules and discharge planning.
Supervises PTA's and Rehab Tech/Aide as required for licensure.
