Job summary

HonorHealth in Tempe, AZ seeks a dedicated Physical Therapist to evaluate, treat, and oversee evidence-based therapy for individuals with physical disabilities. You will develop and update plans of care in collaboration with patients and families to restore function and improve daily living skills.

This full-time day shift role requires BLS, AZ state PT license, and a preference for healthcare teamwork. You will work with physicians, case managers, and the rehab staff to optimize discharge

Qualifications

• Bachelors in Physical Therapy from an accredited university (APTA approved) required.
• Master's or Doctorate in Physical Therapy preferred.
• BLS certification required; AZ PT license active.

Responsibilities

• Implements an individualized treatment plan using evidence-based therapy techniques and provides instructions to patients and families.
• Evaluates patients and develops a comprehensive plan of treatment with measurable goals and discharge planning.
• Maintains accurate documentation and supports billing procedures within established timeframes.
• Collaborates with the interdisciplinary team to optimize discharge planning and patient outcomes; supervises therapy assistants as needed.
• Participates in department meetings and contributes to process improvements.
