HonorHealth seeks a Physical Therapist to evaluate and treat patients with functional limitations in inpatient and outpatient settings across multiple medical centers in the Phoenix area.

You will develop care plans, collaborate with physicians and the care team, and ensure high-quality, evidence-based therapy while documenting progress and outcomes. Renewal of licensure and BLS are required.

Qualifications

• Bachelor's in Physical Therapy required before starting employment.
• Master's or Doctorate in Physical Therapy preferred.
• BLS certification required.
• AZ PT license in good standing or PT Compact license upon hire.

Responsabilités

• Implements an evidence-based treatment plan with skilled therapy techniques and provides instructions to patients and families.
• Evaluates patients and develops a comprehensive plan of care with measurable goals and discharge plan.
• Maintains accurate documentation and billing in the patient record per policy and regulatory requirements.
• Collaborates with the interdisciplinary team to discuss patient status, discharge needs, and equipment requirements.
• Contributes to department needs, recruitment, and staff development through positive role-modeling.
