• Develops and updates plans of care in collaboration with patients/families to restore function.
• Coordinates with physicians, nurses and case managers to ensure appropriate discharge planning and handoffs.
• Documents evaluations, treatment plans, progress notes, discharge summaries, and billing information with accuracy.
