Responsibilities

• Evaluate patients and develop a comprehensive care plan with modalities and education.
• Collect and interpret data from medical records and family.
• Set functional goals and provide evidence-based interventions with ongoing assessment.
• Document evaluations, tests, progress notes, and discharge summaries.
• Coordinate care with physicians, referrals, and rehab staff; supervise PTA and rehab tech.
