Job summary

Advocate Health is seeking a Physical Therapist for our High Point, NC outpatient rehabilitation team. You will provide patient care assessment and treatment, document evaluations and outcomes, and collaborate with a multidisciplinary team to ensure high-quality care.

Responsibilities include ensuring adherence to standards, maintaining productive schedules, and participating in process improvements while supporting patient and family education.

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of practice.
• CPR/BLS Level C certification.

Responsibilities

• Adheres to hospital policies and procedures and promotes teamwork.
• Provides patient care assessment and treatment with sound professional judgment.
• Maintains accurate documentation of evaluations, progress notes, and discharge summaries.
• Meets department productivity and budget expectations and adjusts schedules to patient needs.
• Participates in process improvement and shares best practices to improve patient care.
