Job Opportunity: Outpatient Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Outpatient Physical Therapist, PT for full-time outpatient visits in assisted living communities through our Senior Living office located in Falls Church, VA. The role services adult and geriatric clients across territories in Alexandria/Arlington, VA.

As an outpatient Physical Therapist, you will be a vital part of a multidisciplinary healthcare team, providing outpatient services that enable clients to age in place.

Responsibilities:

1. Make outpatient visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
6. Accurately document all observations, interventions, and evaluations using electronic medical records on a tablet.

Qualifications:

1. Current VA Physical Therapist (PT) license.
2. At least one year of recent PT work experience.
3. Graduation from an American Physical Therapy Association-approved program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computers; experience with EMR systems preferred.
