Outpatient Physical Therapist - Senior Living Visits

We are seeking an experienced Outpatient Physical Therapist, PT, for a Full-Time position throughout Baltimore County, Maryland.

This role involves servicing seniors under their home health benefit within 1-2 communities—home health without the typical commute.

As a home care Outpatient Physical Therapist, you will be a vital part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to help clients remain at home.

Requirements: One year of clinical experience as a licensed Physical Therapist.

Responsibilities:

• Make outpatient visits to clients in designated areas.
• Perform diagnostic tests and measurements such as mobility, joint range, stability, ambulation, muscle strength, balance, and safety assessments.
• Develop and implement individualized care plans, including manual exercises, gait training, and balance interventions.
• Assess and revise care plans regularly, collaborating with nursing staff.
• Educate clients and families on rehabilitative activities to promote health and independence.
• Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications:

• Current Maryland Physical Therapist license.
• Minimum one year of recent clinical experience.
• Graduation from an accredited program by the American Physical Therapy Association or similar.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
