Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing therapeutic care for assigned patients in coordination with physicians and the care team. Responsibilities include patient assessments, developing and implementing care plans focused on goals, engagement, and positive experiences. The PT delivers or oversees most of the patient’s physical therapy across the care continuum.

Key responsibilities:

• Perform evaluations and treatment within legal and ethical standards.
• Assess therapeutic, rehabilitative, and functional status; contribute to care planning.
• Provide skilled therapy at patients’ homes, applying procedures to improve conditions per physician’s plan.
• Educate patients, families, and caregivers on therapies, exercises, and devices for outside sessions.
• Manage caseload, staffing needs, and coordinate patient handoffs.

Qualifications:

• Basic Cardiac Life Support (BLS) certification within 30 days of employment.
• Licensed Physical Therapist.
• Doctorate or Master’s Degree in Physical Therapy.
