BAYADA Home Health Care - Physical Therapist (PT) Opportunity

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time outpatient home health position. The role involves providing outpatient home health visits for our senior living office, serving adult and geriatric clients in Montgomery County's independent and assisted living communities.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home safely and independently.

Responsibilities:

1. Conduct outpatient visits within designated geographic areas.
2. Perform diagnostic assessments, including mobility, joint range of motion, stability, ambulation patterns, muscle strength, endurance, balance, and safety evaluations.
3. Create and update individualized care plans with interventions like manual therapy, gait training, and balance exercises.
4. Collaborate with nursing staff to revise care plans as needed.
5. Educate clients and their families on rehabilitative activities to promote health and independence.
6. Document all client interactions and care activities accurately using electronic medical records on a tablet.

Qualifications:

1. Current Maryland Physical Therapist (PT) license.
2. At least one year of recent outpatient PT experience.
3. Graduation from an American Physical Therapy Association-approved program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Computer proficiency; experience with EMR systems preferred.
