Job summary

gobravvo is seeking a Primary Care Physician (PCP) in Delray Beach, FL for a full-time outpatient role focused on a senior population with complex chronic diseases. The position emphasizes value-based care, collaborative teamwork, and a patient-centered approach in a physician-led environment.

Responsibilities include seeing 18–22 patients daily, managing care plans for seniors with 3–5 chronic conditions, and coordinating with in-house specialists and holistic services.

Qualifications

• Requires residency training in IM, FM, or Geriatric Medicine.
• Board certification or eligibility in FM, IM or geriatrics expected.

Responsibilities

• See 18-22 patients per day with a small panel.
• Develop individualized care plans for seniors with 3-5 chronic conditions.
• Utilize value-based care to improve outcomes and satisfaction.
• Work Monday through Friday, 7:30 am–5:00 pm (last patient 4:15 pm).
• Collaborate with in-house specialists and holistic services.
