Qualifications

• Qualified Mental Health Professional or at least one year of work experience in a mental health setting.
• Enrollment in a Master’s Level Program from an accredited university is preferred.

Responsabilités

• Triages and manages referrals for Mental Health Service Line.
• Inputs referrals into the medical record and tracking system.
• May provide non-contact clinical mental health interventions under direct supervision.
• Facilitates comprehensive care management for bio-psychosocial aspects of illness, trauma, and disability.
• Assists providers with case management needs and crisis intervention response.
• Completes screening tools for front-end assessments.
• Maintains records, statistics, and reports.
• Answers intake line with strong customer service to community stakeholders requesting admission.
• Coordinates with internal systems to move patients through admission.
