Job summary

KAISER in Kapolei, Hawaii, seeks a Licensed Practical Nurse to provide direct and indirect nursing care in an outpatient setting under RN supervision and practitioner orders. You will orient and support LPN Clinic/Meds, perform basic procedures, administer treatments and vaccines, and document patient responses in the EHR system.

You will coordinate with RNs and Practitioners to adjust care plans, manage vaccine data, ensure infection control, and educate patients across varying age groups.

Responsibilities

• Reviews and prepares medical records for assigned patients and initiates follow-up.
• Implements basic nursing procedures, administers treatments and medications as prescribed.
• Documents orders, findings and patient responses in records and informs RN/practitioners of data updates.
• Demonstrates competency in basic nursing skills and participates in staff meetings and projects.
• Performs in emergency clinic situations and assigns tasks to unlicensed personnel as appropriate.
• Follows discharge plans, processes referrals, and provides patient instruction.
• Manages vaccine data, updates EHR/HIR and maintains vaccine inventories.
• Stores and administers vaccines according to guidelines and maintains related paperwork.
• Trains new/existing LPNs on procedures and coordinates with state programs for vaccination inventory.
• Ensures infection control and follows medication storage procedures; provides patient education.
• Participates in orientation of new personnel and maintains age-appropriate care knowledge.
