Qualifications

• Entry level for RNs with minimum of 2 years of Nephrology Nursing experience

Responsabilités

• Performs essential functions under supervision of Supervisor and with guidance from Clinical Educator/Preceptor or collaboration with RNs.
• Performs ongoing collection and analysis of patient data during pre–, during–, post‑hemodialysis treatment and documents in the patient record.
• Modifies treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed.
• Assesses learning needs of patient/family and provides initial and ongoing education.
• Directs daily delivery of safe and effective hemodialysis therapy with the care team.
• Administers medications as prescribed or via approved algorithms; documents justification and effectiveness.
• Initiates or assists with emergency response measures.
• Serves as a resource, coach, mentor and role model for new and incumbent staff.
• Ensures correct laboratory collection, processing and shipping procedures; reschedules missed collections.
• Updates nursing assessment and plan of care in collaboration with Interdisciplinary Team.
• Educates patient about transplant and treatment modality options.
• Participates in facility and area‑level education and quality improvement projects.
• May serve as Preceptor to new employees.
• Completes CAP requirements to maintain or advance.
• Performs other duties as assigned by Supervisor.
