Job summary

Fresenius Medical Care is seeking an outpatient RN CAP 2 to coordinate and deliver high-quality care for patients undergoing hemodialysis. The role includes teaching patients, leading teams, and contributing to quality initiatives.

The RN CAP 2 designation may be entry-level for new hires or earned through the Clinical Advancement Program, with responsibilities across assessment, planning, and evaluation within the ESKD care team.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients.
• Perform ongoing data collection and analysis of patient data during hemodialysis and adjust treatment plans as needed.
• Educate patients and families on learning needs and care plans.
• Direct and provide safe delivery of hemodialysis therapy in collaboration with the care team.
• Serve as a preceptor or mentor for new staff and participate in quality improvement activities.
