Job summary

Fresenius Medical Care is seeking an experienced RN for the Outpatient RN CAP 2 track, offering an entry path into the Clinical Advancement Program for eligible new hires. The role provides and coordinates clinically competent care for an assigned patient group.

You will collaborate with RNs, LVN/LPNs, and Patient Care Technicians, participate in decision making, teaching, and quality improvement activities within the End Stage Kidney Disease team, and support facility operations.

Qualifications

• RN CAP 2 entry path or CAP 1 advancement eligibility.
• Current license to practice nursing in the state.
• CPR/BLS certification maintained as required.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation.
• Document data collection and patient care in the medical record and adjust treatment as needed.
• Educate patients and families on care plans and transplant options.
• Lead or support daily delivery of hemodialysis therapy with the care team.
• Administer medications per protocol and document effectiveness.
• Participate in emergency response and quality improvement activities.
• Serve as resource and mentor to new staff and teammates.
