Job summary

Fresenius Medical Care is seeking an Outpatient RN CAP 2 to provide and coordinate clinically competent care for an assigned patient group. You will assess, plan, intervene, and evaluate care and may delegate tasks to RNs, LVNs/LPNs, and patient care technicians.

The role participates in decision-making, teaching, leadership, and quality improvement activities within the End Stage Kidney Disease care team, under supervision and in collaboration with the Clinical Educator and other staff.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.

Responsibilities

• Assesses and documents patient data during hemodialysis and adjusts care plan as needed.
• Educates patients and families on learning needs and treatment options.
• Directs safe and effective delivery of hemodialysis therapy with the care team.
