Job summary

Fresenius Medical Care is seeking a professional registered nurse for the Outpatient CAP 1 program. This entry level designation provides care coordination, assessment, planning, intervention and evaluation for an assigned group of patients undergoing hemodialysis.

The role supports the End Stage Kidney Disease team and involves collaboration with RNs, LVN/LPNs, and technicians. As part of the CAP, you will educate patients and families, participate in quality improvement activities, and may

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state RN licensure.
• CPR BLS Certification current.
• Must meet state practice requirements in the employed state.

Responsibilities

• Performs data collection and analysis during dialysis and documents in medical record.
• Educates patients and families on care and treatment options.
• Delivers safe hemodialysis therapy in collaboration with staff.
• Administers prescribed medications per protocol and documents effectiveness.
• Assists with emergency response measures and staff training as needed.
• Ensures proper laboratory collection and sample processing.
