Qualifications

• Current state RN licensure.

• Experience in hemodialysis (preferred).

Responsibilities

• Perform ongoing data collection and analysis for dialysis patients and document findings.
• Coordinate and deliver safe hemodialysis care with the care team.
• Educate patients and families about treatment plans and modalities.
• Collaborate with interdisciplinary team to update care plans and outcomes.
