Job summary

Qualifications

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Perform ongoing data collection and document in patient medical record during dialysis, adjusting plan as needed.
• Educate patients and families on learning needs and treatment options.
• Administer medications as prescribed or with approved algorithms and document effectiveness.
• Collaborate with the care team to ensure safe, high-quality hemodialysis delivery.
