The role requires a Bachelor's in a human services field and may require CSAC credentials within 2 years; travel locally is expected, and a valid driving record is required. Salary range is $43,022.07 to $45,890.21.

Qualifications

• Bachelor's degree in acceptable human services field required or a Bachelor's degree in an unrelated field with 90 credit hours of human services courses.
• CSAC credential is preferred at time of hire; if not, must complete requirements within 2 years of hire.
• Ability to transport clients and maintain a good driving record.

Responsibilities

• Provide education, assessment and discharge planning services to clients at high risk of relapse/continued use if basic needs are not met.
• Engage clients in needed services and coordinate supports across medical, psychiatric, social, educational, vocational and residential settings.
• Monitor and document progress to improve outcomes in addressing substance use, mental health and co-occurring issues.
