Qualifications

• Education: Minimum of a high school diploma; an Associate's or Bachelor's in human services preferred.
• Experience: Clinical experience in human services with children/adolescents is preferred.
• Licensure/Certifications: QMHP preferred.
• Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry reported upon selection.
• Safe Driving Record: Driving clients; no more than two moving violations in 3 years; no criminal moving violations in 5 years.

Responsibilities

• Develop comprehensive care plans for assigned clients.
• Provide case management services based on identified client needs.
• Help clients and families overcome barriers to services.
• Make phone calls to assess needs and coordinate services; attend office and school meetings.
• Conduct home and school visits; participate in doctor appointments.
• Collaborate with community agencies; educate families about available supports.

Education

High school diploma

Associate's or Bachelor's degree in human services (preferred)
