Job summary

Blue Ridge Behavioral Healthcare, Roanoke, VA, is seeking a Case Manager/Liaison to coordinate care for adults with substance use disorders and to provide discharge planning for MAT clients in the local detention center and in the community. The role includes educating residents on MAT benefits, providing REVIVE training, and assisting with MAT initiation before release and during community transition.

A Bachelor's degree in a human services field is required, with CSAC credentialing eligibility

Qualifications

• Bachelor's degree in an acceptable human services field is required or bachelor in other field with minimum 90 hours of human services field classwork.
• Preferred one year of direct experience providing services to individuals with substance use disorders.
• Must meet the requirements for qualification as a Substance Use Disorder Case Manager (SUD CM) and be eligible for supervision toward Certified Substance Abuse Counselor (CSAC) credentialing.
• Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
• Safe Driving Record: This position requires transporting clients or driving agency vehicles; no more than two moving violations in the last three years and no criminal moving violations in the last five years.

Responsibilities

• Provide case management for Adult Clinical Services clients identified with an Opioid Use Disorder (OUD) or other Substance Use Disorder.
• Discharge planning for those on MAT in the local adult detention center and linkage to continuation of MAT services in the community.
• Educate residents of the local jails to the benefits of MAT and provide REVIVE training.
• Help residents learn about MAT and begin medication before leaving the detention center and during transition to the community.
