Position Summary/Responsibilities

The Case Manager/MAT Liaison provides case management for Adult Clinical Services clients identified with an Opioid Use Disorder (OUD) or other Substance Use Disorder. Provides information and discharge planning for those on MAT in the local adult detention center for residences who may benefit from participation in Medicated Assisted Treatment (MAT) services or linkage to continuation of MAT services in the community. They will also help to educate the residents of the local jails to the benefits of MAT and provide REVIVE training. The Case Management/MAT Liaison will serve to help educate and provide services that will help residents to learn about MAT and help them to begin the medication before leaving the adult detention center as well as during the transition to the community. Hiring Range

The anticipated base salary for this position is $43,022.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.

Requirements

• Education: Bachelor's degree in an acceptable human services field is required or bachelor in other field with minimum 90 hours of human services field classwork.
• Experience: Preferred one year of direct experience providing services to individuals with substance use disorders, including individuals with co-occurring substance use and mental health disorders.
• Licensure/Certifications: Must meet the requirements for qualification as a Substance Use Disorder Case Manager (SUD CM) and be eligible for supervision toward Certified Substance Abuse Counselor (CSAC) credentialing.
• Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
• Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.

Special Work Conditions

This role requires the ability to communicate effectively with staff, clients, and partners, both verbally and in writing. The employee must be able to remain in a stationary position for portions of the workday, move throughout office or community settings, and operate a computer and standard office equipment. The position may occasionally require lifting or transporting materials or supplies up to 10 pounds. This role may require participation in trauma-informed physical practices, such as standing, positioning, and de-escalation techniques, when such activities are essential to the position. These functions may be performed with or without reasonable accommodations.
