Job summary

Southwest Behavioral & Health Services is seeking a dedicated Case Manager for our Flagstaff team. You will conduct assessments, coordinate care, and connect members with community resources, primarily serving adults within an interdisciplinary team.

Hourly pay starts at $20, with benefits including PTO, holidays, health/dental/vision coverage, disability and life insurance, and a strong 40% retirement match program. This role emphasizes growth and internal promotion.

Qualifications

• Minimum age 22.
• Bachelor’s degree in the behavioral health field plus one year of relevant experience.
• A combination of experience and education totaling 2 years is acceptable.
• Valid AZ driver’s license and motor vehicle clearance.
• Fingerprint clearance issued by the Arizona Department of Public Safety or eligible to receive upon hire.
• 3 weeks of PTO your 1st year of employment, with increased accruals after continued service.

• Intake screenings (Completing intake paperwork and referrals)
• Completing annual behavioral health assessments, including service planning and support planning
• Facilitating CFTs and ARTs
• Identifying gaps in care for our members
• Completing SMI and SED evaluation
