Job summary

Southwest Behavioral & Health Services is seeking a dedicated Case Manager for Supported Housing in Flagstaff. You will coordinate intakes, assessments, leases, and housing stability while ensuring HUD compliance and resident well-being. Strong communication, organization, and problem-solving skills are essential.

Pay rate is $20/hr. The role involves daily interaction with residents and coordination with landlords and service providers to support recovery-based housing.

Qualifications

• Bachelor’s degree and 1 year health experience or five years related work.
• Housing or case management experience with underserved populations preferred.
• Negative TB results required, fingerprint clearance, and MVR as applicable.

Responsibilities

• Perform intakes, screening and assessments for residents.
• Manage leases, notices, terminations, and housing stability for caseloads.
• Ensure HUD compliance, Fair Housing, and ADA accommodations.
• Oversee unit safety, inspections, and repairs coordination.
• Document services and perform income eligibility calculations.
